Important Information About Appointments
To request an appointment, go to our Appointments page for complete information or to complete an online appointment request form. The doctors participate in many HMO's and PPO's. It is important that you know before you see a doctor whether he is included in your plan. We will be happy to give you any information before the day of your appointment. Also, before the day of your appointment please furnish us with the authorization numbers from primary care physicians.
Please drink several large glasses of water before coming to our office, as this will allow you to provide a urine specimen.
Your appointment will be with the doctor of your choice. He will see you and follow your treatment. However, particularly when hospitalized, you may be seen by all of our doctors, and they will participate in your care.
The office works on an appointment basis. We make every effort to adhere to our schedule; however, because of office and hospital emergencies and telephone calls, we occasionally get behind. Every effort will be made to notify of a delay. Sometimes patients must be rescheduled. We request your cooperation if this occurs.

What to Bring to Your Appointment
- Your insurance card
- Physician referral forms or authorization numbers if required by insurance
- A list of current prescription or over-the-counter medications you are taking, including dose and frequency
- Pertinent information about your medical and surgical history
- Any pertinent x-rays or records you may have

Pediatric Forms
Forms completed and typewritten in advance provide the following benefits:
- We receive more accurate and legible records from you.
- We are able to prepare for a more individualized patient-doctor visit.
- You save time by not having to complete the form when you arrive at the office.

Payment and Insurance
Dallas Urology is a provider for Medicare and most major insurance plans. We also belong to many HMO's and PPO's that are listed below. We provide insurance billing.
We request payment at the time of your appointment for services that are not covered by insurance. When necessary, our staff will work closely with patients who require a payment plan.
If you have any questions regarding which insurance plans we accept or any patient billing concerns, please call us. Questions regarding your coverage and benefits should be directed to your employer or insurance company.
